Dear Parents: Lincoln Elementary would love to hear from YOU! This is the last week to give your important input. Please help us to grow and improve to build better learners. You have been invited to complete the Illinois 5Essentials Parent Survey. The information you provide will help guide improvement as we strive to inspire each student to realize their potential through the education and opportunities we provide at Lincoln Elementary School. Your participation in this survey is voluntary but crucial. We can’t receive results from the survey unless at least 20 percent of parents respond. The more responses parents provide, the more we will know how to focus our efforts toward the improvements you want. Therefore, we have set a goal to reach an 80 percent response rate. Please help us reach our goal by completing the survey. • Go to: http://survey.5-essentials.org/illinois/survey/parent/ to take the survey. • You may enter your email address if you would like to receive confirmation of your submission, but it is not required. • Then enter “LaSalle” into the county field and “Lincoln Elem School-Ottawa” to find our school. You will then be asked a series of questions to provide input regarding your family’s experience here at Lincoln Elementary School. Please remember to click “submit” once you are done so your responses can be counted. Please know that your identity and responses will be kept confidential and will not be connected with your child. If you have multiple children who attend Lincoln, you may complete one survey for each child. However, only one parent per household may submit a survey for a given child. The survey is open now and closes on March 28, 2025, so please submit your responses soon! When a parent completes the 5Essentials Survey, please let your child's teacher know via School Status that you have completed it and your child will recieve $10 in Lincoln Loot! In addition, When you have completed the 5Essentials survey, please notify your classroom teacher or the Lincoln office and your name will be placed in a raffle for a Lotto scratch off basket in addition to your child receiving $10 in Lincoln Loot. We are looking forward to hearing from you! We will draw a name for the raffle when the 5Essentials survey window closes on March 28. Thank you in advance for your time and input. We appreciate you!

Dear Parents, This is a reminder to participate in the parent survey supplement, part of the 5Essentials Suvery administration. We guarantee that survey responses will never be connected to the identity of you or your child. School reports will only present aggregated information of how parents and students responded as a whole. A parent survey report will be generated if at least 20% of parents/guardians respond to the survey. While the parent data will not be made public, the parent report will be made available to Lincoln Elementary Administration, as well as the Ottawa Elementary Distict office and will enable us to make future decisions based on the responses given be Lincoln families. Your participation in the parent survey will help us understand the conditions at your child's school and guide future improvements. Again, you identity and individual survey responses will be kept completely confidential and will never be connected to you or your child. Teachers and 4th - 12th grade students throughout Illinois are taking the 5Essentials Survey. School-level 5Essential Reports will be generated if teachers and/or students meets the 50% response-rate thresholds. These research-based reports will be sent to schools and districts in May and will also be included in ther State School Reports Card, released statewide in the fall. Please visit http://survey.5-essentials.org/illinois/survey/parent/ to take the parent survey. If you have any questions about the survey, please contact UChicago Impact Survey Support at 1-866-440-1874 or impact-surveys@uchicago.edu When a parent completes the 5Essentials Survey, please let your child's teacher know via School Status that you have completed it and your child will recieve $10 in Lincoln Loot!
